NMC testosterone hormone replacement therapies vary according to (1) the method of distraction; the type of custom compounded bio-identical testosterone medication administered (gel, cream or injections); and (3) the duration of the patient's HRT treatment period. All National Medical Clinic, Inc. hormone replacement therapies for men include the continuous administration of custom compounded generic Anastrozole to prevent the aromatase conversion or the patient's increased testosterone level from being converted to estrogens during treatment,

  1. National Medical Clinic™ 'Alternating (Cycled) Testosterone and HCG with Continuous Anastrozole Hormone Replacement Therapy for Men'. This method of male hormone replacement therapy (HRT) consists of alternating (cycled) 10 to 12 weeks of testosterone administration followed by 2 weeks of HCG administration in each of several consecutive testosterone and HCG treatment cycles consisting of 12 to 14 weeks during a patient's total male hormone replacement therapy treatment period. There are two or more testosterone and HCG treatment HCG cycles ranging from 12 to 14 weeks in any treatment period. The number of testosterone and HCG treatment cycles increase as the duration of the patient's total treatment period is extended. Custom compounded Anastrozole tablets are administered periodically each week throughout hormone replacement therapy to prevent the aromatase conversion of increased testosterone in the blood stream resulting from the administration of testosterone medication to estrogens.
  2. National Medical Clinic™ 'Continuously Administered Testosterone, HCG and Anastrozole Hormone Replacement Therapy for Men'. This NMC method of male hormone replacement therapy consists of the continuous administration of testosterone cypionate, gel or cream, HCG and Anastrozole periodically during each week throughout the patient's HRT treatment period. If the patient is administering testosterone gel or testosterone cream once daily, then the only HCG and Anastrozole are administered on the same two days during each week throughout the duration of the patient's NMC HRT program. The fees for the continuously administered testosterone HCG and Anastrozole hormone replacement therapies for men are approximately $400.00 more than the alternating (cycled) administration of testosterone and HCG with continuous Anastrozole administration HRT for Men with comparable treatment durations described immediately above because the continuous testosterone, HCG and Anastrozole hormone replacement therapies for men require a greater volume of medications throughout the treatment period; more complex evaluation of patient lab test reports while the patient undergoing HRT; more difficult dose rate adjustments; and continuous male HRT consumes more NMC administrative and medical staff time to properly administer a complex, but highly effective medical treatment.

The primary benefit of the continuous administration of testosterone, HCG and Anastrozole medications throughout the period of male hormone replacement therapy in comparison to the alternating or cycled administration of testosterone 10 to 12 weeks followed by 2 weeks HCG administration is that a portion of the patient's increased daily testosterone production and resulting higher testosterone blood level during treatment is produced naturally by the testes in response to the continual twice weekly stimulation of the testes by HCG injections, rather than the entire increase in patient testosterone production and testosterone blood level in the blood stream resulting from the administration of testosterone medication only for periods of 10 to 12 weeks followed by 2 weeks of HCG administration only.

An additional treatment benefit of continuously administering testosterone, HCG and Anastrozole during male HRT in contrast to alternating or cycled administration of testosterone for 10 to 12 weeks without HCG until the final 2 weeks of each testosterone and HCG 12 to 14 week cycle is that continuous administration of the HCG under the continuous method of administering all three medications during throughout the period of HRT reduces the likelihood of any reduction in natural testosterone production by the testes due to an endocrine system negative response to the higher than normal level of testosterone in the patient's blood stream causing a reduction in the volume of Leutenizing Hormone (LM) produced and released by the pituitary gland to stimulate the testes to produce natural testosterone. The endocrine system has a tendency to reduce natural testosterone production as a means for compensating for the higher than normal level of testosterone in the blood stream resulting from the administration of testosterone medication only for 10 to 12 weeks. This is because the male endocrine response by HPTA (hypothalamic-pituitary-testicular axis) to the above normal plasma testosterone level seeks to maintain the normal genetic level of testosterone in the patient's blood stream for the patient's age that the testes currently produce in response to LH secretions without the addition of testosterone medication or HCG further stimulation of the testes to cause the testes to produce more natural testosterone and thereby elevate plasma testosterone level in the blood stream above the normal or genetic testosterone level for male patient undergoing HRT by means of the administration of both testosterone natural testosterone production. This normal or genetic level of testosterone in the blood stream declines with aging or males.

National Medical Clinic, Inc. hormone replacement therapy protocols for men include the male patient's administration of HCG during testosterone hormone replacement therapy to protect the patient against loss of natural testosterone production by the testes and testicular atrophy due to an endocrine response by HPTA (hypothalamic-pituitary-testicular axis) that responds to an increase of testosterone in the blood stream above a specific male's normal or genetic testosterone level for his age by reducing LH (Leutenizing hormone) production and secretion by the pituitary gland. As the volume of LH produced and released by the pituitary gland in the brain declines the volume of LH stimulating the testes to produce natural testosterone also decreases. And this endocrine system process results in a reduction in the volume of natural testosterone by the male testes as the endocrine system functions to counterbalance the increased level of testosterone the hypothalamus gland in the brain measures in the blood stream.

HCG protects the patient against this loss of natural testosterone production due to his own endocrine system response while on testosterone and HCG hormone replacement therapy. The natural decline in male testosterone production that occurs with aging is attributed to a decline in the gonadal stimulating pituitary hormone LH (Leutenizing hormone). As a result of the hypothalamus secreting less gonadoropin-releasing hormone (GhRH), which stimulates the pituitary gland to produce LH, the pituitary gland reduces the production of LH. This decrease in the pituitary secretion of LH reduces the stimulation of the gonads or male testes and results in declining testosterone and sperm production due to the decreased function of the gonads. The decreased stimulation of the testes by the pituitary's diminished secretion of LH can also cause testicular atrophy. HCG functions like naturally produced LH in a male and stimulates the testes in the same manner as naturally produced LH. This stimulation of the testes by HCG interrupts the negative feedback signal generated by the HPTA (hypothalamic-pituitary-testicular axis) endocrine response to an increase in plasma testosterone in the blood stream above a specific male's normal or genetic testosterone level for his age by reducing LH (Leutenizing hormone) production and secretion by the pituitary gland. As a result of the interruption of this negative feedback signal sent from the hypothalamus in the form of reduced gonadoropin-releasing hormone (GhRH) delivered to the pituitary gland in the brain, the male patient undergoing hormone replacement therapy that includes HCG administration does not sustain a loss of increased natural testosterone production during HRT or testicular atrophy due to HCG stimulation of the testes in the same manner as the declining LH with the result that natural testosterone production by the testes does not decline as plasma testosterone level rises in the blood stream during male hormone replacement therapy. The administration of HCG during male testosterone hormone replacement therapy uses the body's own biochemical stimulating mechanisms to increase plasma testosterone level in the blood during HRT. National Medical Clinic, Inc. (NMC) also offers an HCG therapy to increase natural testosterone production in men to stimulate the testes of men who are hypogonadal or lack sufficient testosterone. NMC physician protocols for HCG therapy to increase natural testosterone production in hypogonadotropic men include supplemental testosterone medication and Anastrozole. For the forgoing reasons, the continual administration of HCG to stimulate the testes and interrupt the negative feedback signal generated by the HPTA (hypothalamic-pituitary-testicular axis) endocrine response to an increase in plasma testosterone in the blood stream above a specific male's normal or genetic testosterone level by reducing LH (Leutenizing hormone) production and secretion by the pituitary gland causing a decrease in natural testosterone production during HRT more effectively prevents a declined in the natural testosterone production and plasma testosterone level in the blood stream than the administration of HCG in only the last two weeks of a twelve to fourteen week period of alternating (cycled) male hormone replacement therapy.

DEFINITIONS

Human chorionic gonadotropin (HCG) is a medication administered to protect the patient from the decline or total loss of natural testosterone production by the testes and testicular atrophy. HCG is combined with 'Testosterone Therapy for Men' by the doctors of National Medical Clinic, Inc. to protect the patient against loss of natural testosterone production by the testes and testicular atrophy due to an endocrine response by HPTA (hypothalamic-pituitary-testicular axis) that responds to higher than normal level of testosterone in the patient's blood stream by reducing LH and the production of natural testosterone by the male testes. Absent HCG administration in combination with testosterone hormone replacement therapy, the patient's endocrine's system's response to the patient' higher than normal low level of testosterone during therapy would result in a declining level of the patient's natural testosterone production over a sustained time period until the natural production ceased entirely.

'Anastrozole' is an aromatase inhibitor that is administered to men undergoing testosterone therapy to block the conversion of testosterone to estrogen and the body's inefficient utilization of testosterone medication and testosterone produced naturally by the testes. Generic Anastrozole is custom compounded in small dose tablets and prescribed for NMC patients by the treating doctors of National Medical Clinic™ no additional cost when a patient's 'Estradiol' level is high and out of range on any patient lab test taken while the patient is undergoing an NMC testosterone and HCG hormone replacement therapy.

PATIENT LABORATORY BLOOD TESTING AND THE TREATING DOCTOR'S ADJUSTMENT OF PATIENT'S DOSE RATES AS INDICATED BY SUCH LAB TESTS ARE THE MEANS OF ACHIEVING NMC'S TWO (2) PRIMARY TREATMENT OBJECTIVES WHILE A MALE PATIENT IS UNDERGOING TESTOSTERONE HORMONE REPLACEMENT THERAPY

Each NMC testosterone and HCG hormone replacement therapy for men includes two (2) paid 'free testosterone' lab tests at the end of the 4th and 8th weeks of therapy and one (1) 'Estradiol' lab test at the end of the 4th week of treatment while a male patient is undergoing hormone replacement therapy (HRT) to determine the proper dose rates for the therapy medications being administered to achieve the above referenced two (2) NMC hormone replacement therapy (HRT) treatment objectives contained in the applicable "NMC Treatment Protocol and Patient HRT Instructions".

Visa, MasterCard and Discover credit card and debit card payments are accepted by American Medical General, Inc. and National Medical Clinic, Inc. as payment of a medical retainer fee for the patient requested medical treatment assessment and indicated medical treatment at the time of enrollment. Payment may be made in two (2) equal installment payments made 30 days apart with a recurring credit card charge, or as a single one-time payment that will save the patient $200.00, at the time of patient telephone enrollment. If initial screening lab tests or the patient's physical examination and evaluation by the NMC independent medical provider determine the treatment sought by the patient is not indicated, then the balance in the patient's medical retainer account is refunded to the patient by AMG. If treatment is indicated and prescribed by the treating NMC medical provider, medications are dispensed directly to the patient upon receipt of payment from AMG by an independent pharmacy to the patient upon AMG's receipt of the second installment payment of the medical retainer fee from the patient.

There is an additional administrative fee payable to American Medical General, Inc. at the time of submission of a request for insurance claim information, but not coding data, if the patient's request is accepted by NMC. Acceptance of such a request by NMC depends upon the time and resources required, cost of rendering such administrative services and complexity of the undertaking. Providing information acceptable to medical insurers can be very time consuming and therefore costly to provide patients.

Alternating Administration of Testosterone and HCG Therapy with Continuous Anastrozole Male Hormone Replacement Therapy (HRT) consists of alternating 10 to 12 weeks of testosterone administration followed by two weeks of HCG medication in each testosterone and HCG treatment cycles during the treatment period. There are 2 or more testosterone and HCG treatment HCG cycles ranging from 12 to 14 weeks in any treatment period. If the patient is administering testosterone gel or testosterone cream once daily, then HCG and Anastrozole are administered periodically each week at the same time throughout the treatment period. The continuous administration of the three medications throughout the period of hormone replacement therapy are slightly more costly than the alternating (cycled) administration of testosterone and HCG therapies reference above because they require a grater volume of medications throughout the treatment period, as well as, increased administrative and medical staff time to administer. The primary benefit of the continuous administration of testosterone and HCG throughout the treatment period in comparison to the alternating or cycled administration of testosterone and HCG is that a portion of the increased daily production of testosterone during male hormone replacement therapy is produced naturally by the testes, rather than the entire increase in testosterone production being provided by testosterone medication alone. An additional benefit of continuous administration of testosterone and HCG over cycled administration is that there is a significantly reduced probability of any loss in natural testosterone production by the testes due to an endocrine system negative response to continuous testosterone medication because of continuous stimulation of the testes by HCG.

The NMC all-inclusive total single price for these NMC medical treatments includes the cost for pharmacy compounded HCG and Anastrozole aromatase inhibitor. The aromatase of testosterone to estrogen increases the patient's estrogen blood level and inhibits the volume of free testosterone in the blood stream that is unbound and bio-available for processing by the body. The purpose of the 'aromatase inhibitor' is to prevent the patient's testosterone from being converted to estrogens so that the patient's free testosterone blood level is not reduced by estrogens. Therefore, treatment objectives include elevating the patient's free testosterone level within the normal adult male free testosterone reference range while maintaining the patient's Estradiol blood level within the normal Estradiol reference range for adult males.

The National Medical Clinic, Inc. medical treatment protocol prescribed by your NMC treating doctor and its related "NMC Medical Treatment Protocol for Alternating Administration of Testosterone and HCG and with Continuous Administration of Anastrozole Hormone Replacement Therapy for Men and Patient Treatment Instructions" distributed to you at the beginning of your treatment may provide for alternating self-injection of testosterone medication for a specific period of ten to twelve weeks followed by a two week period of HCG medication injections to protect you from loss of your natural testosterone production. If your patient treatment instructions state that you are to alternate a period of testosterone injections followed by a period of HCG injections, then the time period of your injection of testosterone medication plus the time period of your injection of HCG medication equals the duration of one (1) testosterone and HCG therapy alternating treatment period. A patient will undergo several alternating testosterone and HCG HRT treatment periods during the total HRT treatment period of the NMC alternating testosterone and HCG therapy in which the patient is enrolled. The patient shall also administer Anastrozole tablets throughout the total period of HRT to prevent the aromatase conversion of testosterone to estrogens.

Alternatively, the National Medical Clinic, Inc. medical treatment protocol prescribed by your NMC treating doctor and its related "NMC Medical Treatment Protocol for Concurrent Administration of Testosterone, HCG and Anastrozole Male HRT and Patient Treatment Instructions" distributed to you at the beginning of your treatment may provide for your concurrent self-administration of testosterone, HCG and Anastrozole at the same time twice weekly, except that testosterone gel or cream is applied once daily rather than bi-weekly with the HCG and Anastrozole.

In addition to the above referenced Alternating Administration of Testosterone and HCG with Continuous Anastrozole Hormone Replacement Therapy for Men, NMC also offers twelve (12) state-of-the-art, cutting-edge, high quality and effective 'Continuously Administered Testosterone, HCG and Anastrozole Hormone Replacement Therapies for Men'. Additional services requested by a patient enrolled in this NMC medical treatment not included in the above medical treatment description may be provided by an NMC or AMG staff member, Medical Counselor or Patient Services Director at the stated billing rate per fifteen minutes, or any portion thereof, set forth in the patient's enrollment confirmation. An estimated medical retainer is payable by the patient in advance for additional staff services not included in the list of treatment services described above. Such additional services shall not necessarily be available due to resource requirements including personnel staffing and patient case loads in the NMC patient medical services system even though patient is agreeable to paying for such additional staff services.

MEDICAL RETAINER FEE

Payment of Medical Retainer Fee for Treatment Requested

Visa, MasterCard and Discover credit card and debit card payments are accepted by American Medical General, Inc. and National Medical Clinic, Inc. as payment of a medical retainer fee for the patient requested medical treatment laboratory testing, physical examination, clinical assessment and indicated medical treatment at the time of enrollment. Payment may also be made in two (2) equal installment payments made 30 days apart with a recurring credit card charge, or as a single one-time payment that will save the patient $200.00, at the time of patient telephone enrollment. If NMC patient's pre-exam lab blood test report or the patient's physical examination, evaluation and assessment by the independent medical provider to whom the patient is referred by the Physician Referral Service or the National Organization of American Physicians™ determines that the treatment sought by the patient is not indicated, then the balance in the patient's medical retainer account less fees incurred is refunded to the patient's credit card by AMG. If treatment is indicated and prescribed for the NMC patient by the treating medical provider to whom the patient is referred, medications are dispensed and shipped directly to the patient by a U.S. independent compounding pharmacy to the patient upon AMG's confirmation to NMC of AMG's receipt of the second installment payment of the medical retainer fee from the NMC patient if the patient elects to pay in two installment payments.

AMG and NMC do not accept medical insurance assignment of benefits as payment for any NMC medical treatment.

Treatment periods for HCG Therapy to Increase Natural Testosterone Production in Men Supplemented with Testosterone Cypionate, Gel or Cream and Anastrozole Aromatase Inhibitor range from 20 to 50 weeks in duration pharmacy compounded bio-identical HCG injected intramuscularly periodically each week days to stimulate the testes' increased production of natural testosterone.

Treatment also includes exogenous testosterone medication administered to supplement the increase in natural testosterone production resulting from HCG stimulation of the testes to enable the patient to achieve the dual treatment objectives of (1) a patient free testosterone level in the upper 33% of the testing medical laboratory's normal reference range for adult male free testosterone; and (2) an estradiol level within the testing medical laboratory's normal reference range for male estradiol.

The Decline in Gonadal Stimulating Pituitary Hormone LH (Leutenizing hormone)

The natural decline in male testosterone production that occurs with aging is attributed to a decline in the gonadal stimulating pituitary hormone LH (Leutenizing hormone). As a result of the hypothalamus secreting less gonadoropin-releasing hormone (GhRH), which stimulates the pituitary gland to produce LH, the pituitary gland produces declining amounts of LH. This decrease in the pituitary secretion of LH reduces the stimulation of the gonads or male testes and results in declining testosterone and sperm production due to the decreased function of the gonads. The decreased stimulation of the testes by the pituitary's diminished secretion of LH can also cause testicular atrophy. HCG stimulates the testes in the same manner as naturally produced. Hours of operation for patient consultations and enrollment in treatment assessment and indicated treatment are Monday thru Friday 1:00 P.M. to 6:00 PM EST. Our phone lines are answered by staff members or messages are taken 24 hours per day seven days a week. National Medical Clinic, Inc. is primarily responsible for providing NMC patients a comprehensive and effective medical treatment process. American Medical General, Inc. (AMG) is primarily responsible for assuring payment of NMC patient fees to medical laboratories, medical providers, NMC staff and operations personnel and independent U.S. compounding pharmacies for dispensing medications prescribed for NMC patients by independent medical providers to whom the patient is referred for medical evaluation and treatment.

Your registration, enrollment, payment of your medical retainer fee for your sought treatment, diagnostic medical laboratory testing, physical examination and assessment for indicated medical treatment by an NMC independent medical provider is subject to NMC securing a doctor's order for your pre-physical exam lab testing, a low free testosterone level on your lab test report, other medical factors, your medical history, physical exam findings, clinical assessment and diagnosis by the NMC or AMG independent medical provider to whom you are referred and who shall be responsible for prescribing your treatment if indicated and supervising your treatment through lab testing and achieving protocol medical treatment objectives through medication dose rate adjustments.
